Please arrive 10 minutes early to your first appointment with
Dr. Pearson, and bring the following with you --

• Completed new patient information form
• List of current prescriptions and/or over-the-counter medication, including dose and frequency
• Information about your medical history or other relevant info
• Release of information forms with names and contact information of​ any prior or current treatment providers you wish Dr. Pearson to contact
• Completed patient payment information form below

Please ensure that you arrive on time for your appointment. If any necessary paperwork needs to be completed, please arrive at least 5-10 minutes prior to your appointment.

If you are unsure whether you need to complete any paperwork, please arrive early.

IMPORTANT

If you cannot keep your appointment,
please call 617-383-4453 to cancel or reschedule your appointment AT LEAST 24 HOURS IN ADVANCE.

Missed appointments, and appointments cancelled
without 24 hours notice, will be charged Dr. Pearson’s full
appointment rates.

Dr. Pearson is an out-of-network provider for all insurance carriers.

Therefore, you will be billed the full amount of services rendered at the time of your visit.

Dr. Pearson also is happy to provide a receipt which you can submit for possible reimbursement with your
​insurance according to the policies of your health plan.

You will be charged the full fee for sessions cancelled with less than 24 hours notice.
(Please note if you are using insurance that insurance companies do not pay for missed sessions.)
​You will be responsible for this fee.
​
Dr. Pearson accepts checks, cash, Zelle, Paypal, Venmo and credit cards including HSA cards
​(Visa, MasterCard, and American Express).

​PAYMENT IS EXPECTED AT THE TIME OF SERVICE
